NDIS Services
SUPPORT CO-ORDINATION
Services include:
- Guiding participants through the NDIS Process
- Working together to identify goals
- Selecting the most appropriate services to meet their needs
- Supporting participants in budgeting for each type of support and negotiating prices and service agreements
THERAPEUTIC SUPPORTS
Services include:
- Assisting participants to apply and improve their functional skills
- Improve participation and independence in daily, practical activities
- Improve skills and abilities such as language and communication, personal care, mobility and movement, interpersonal interactions and community living
HOME MODIFICATION
Services include:
- Home modification design and construction
- Installation of equipment
- Changes to building structures, fixture or fittings to enable participants to live as independently as possible or to live safely at home
BEHAVIOUR SUPPORT
Services include:
- Addressing the needs of the person identified as having complex behaviours of concern
- Behaviour Management Plan for improved relationships
- Training in behaviour management strategies
PARTICIPATE COMMUNITY
Services include:
- Assisting participants to participate actively in community, social and civic activities
- Includes supporting participants during these activities
- Developing participants’ ability to partake in these Activities
HOUSEHOLD TASKS
Services include:
- Essential tasks such as: meal preparation and delivery
- Assistance with house or yard maintenance, cleaning and linen
EARLY CHILDHOOD SUPPORTS
Services include:
- A mix of therapies
- Supports all children 0-6 years with developmental delay or disability
- Assistance to families to achieve better long-term outcomes
DEVELOPMENT-LIFE SKILLS
Services include:
- Development of daily living and life skills
- Focuses on training and development activities
- Increase their ability to live as independently as possible
INNOVATIVE COMMUNITY PARTICIPATION
Services include:
- Activities not included under other community participation
- Registration groups, to enable participants in access mainstream activities
ASSIST LIFE-STAGE, TRANSITION
Services include:
- Individual employment support
- Employment preparation and support in a group
- School leaver employment supports
- Transition through school and to further education
ASSIST ACCESS MAINTAIN/EMPLOY
Services include:
- Short and long-term support
- Focus on strengthening the participant’s ability to coordinate their supports
- Assist them to live at home and participate in their community
ACCOMMODATION/TENANCY
Services include:
- Transition supports only
COMMUNITY NURSING CARE
Services include:
- Specialist care for participants who have high care needs requiring a high level of skill
- Training of support workers to respond to the participant’s complex Needs
ASSISTANCE WITH TRAVEL/TRANSPORT ARRANGEMENTS
Services include:
- Rental of adapted vehicle
ASSISTANCE WITH DAILY PERSONAL ACTIVITIES
Services include:
- Assisting with and/or supervising personal tasks of daily life to enable the participant to live as autonomously as possible
ASSISTANCE WITH PERSONAL ACTIVITIES HIGH
Services include:
- Assisting with and/or supervising personal tasks of daily life to enable the participant to live as autonomously as possible
- These individual supports can be provided in the participant’s own home, and other environments
